Rename the removed CDK portal directive selectors
ProprietaryCDK 21 removed PortalHostDirective and TemplatePortalDirective. Renames the selectors only they matched to those of CdkPortalOutlet and CdkPortal: [cdkPortalHost] becomes [cdkPortalOutlet], a #ref="cdkPortalHost" reference becomes #ref="cdkPortalOutlet", and <ng-template cdk-portal> becomes <ng-template cdkPortal>, in .html templates and inline template metadata.
